I am a paramedic and clinical mentor, and have worked in the pre-hospital setting for 18 years in three countries. Over the years, I have learnt to critically appraise the work I do, and use reflection as a tool to improve my practice. I now realise that a lot of what I was learning through this process could be attributed to something called “Human Factors”. While I do not consider myself to be a human factors expert, I have gained some experience whilst working for the Patient Safety Federation, project managing the “No Needless Ignorance workstream”. I am happy to discuss incidents that have not gone well in order to highlight how different aspects of human factors have played a role. I believe that both clinicians and the organisations we work for need to take more responsibility in order to improve patient safety.
